𝘗𝘭𝘦𝘢𝘴𝘦 𝘣𝘦 𝘪𝘯𝘧𝘰𝘳𝘮𝘦𝘥 𝘵𝘩𝘢𝘵 𝘵𝘩𝘦 𝘥𝘦𝘢𝘥𝘭𝘪𝘯𝘦 𝘧𝘰𝘳 𝘤𝘰𝘮𝘱𝘭𝘦𝘵𝘪𝘰𝘯 𝘰𝘧 𝘐𝘯𝘤𝘰𝘮𝘱𝘭𝘦𝘵𝘦 𝘎𝘳𝘢𝘥𝘦/𝘴 𝘪𝘯𝘤𝘶𝘳𝘳𝘦𝘥 𝘥𝘶𝘳𝘪𝘯𝘨:

𝑹𝒆𝒎𝒊𝒏𝒅𝒆𝒓𝒔:
1. Submission of completion of grades form will be accomplished on-site.
2. Failure to submit the fully signed form to the Registrar’s Office, the INC (Incomplete) mark will automatically become a FAILING grade.
3. Only student/s with INC (Incomplete) remarks/grades will comply.
𝑷𝒓𝒐𝒄𝒆𝒅𝒖𝒓𝒆:
1. Proceed to the Registrar’s Office and request for Completion of Grades form.
2. Upon completion of the requirements, the instructor shall give the completed grade/s with his/her signature.
3. Have duly signed by the Department Head approval.
4. Submit the accomplished form to the Registrar’s Office to update the records accordingly.
